1. Print, fill out and mail the enrollment form below.

2. Include your nonrefundable deposit of $200 towards your tuition. You may pay this deposit via credit card (Visa, Mastercard, AmEx or Discover), paypal.com or cash in person.

3. If other arrangements are not made, the balance of your tuition is due in full two weeks prior to your start date to avoid "No Shows".

4. Students must be 16 or older on the first day of class.
